Breach Notification
In the unlikely event of a breach of unsecured PHI, we are committed to complying with all HIPAA breach notification requirements. We will notify affected individuals promptly, no later than 60 days from the discovery of the breach, and will also notify the Secretary of Health and Human Services and the media when required by law.
